Qlogic FCA 2214 errors in Win2000/3 eventlog
I get lots of EventID: 9, Source: ql2300.
The device, \Device\Scsi\ql2300, did not respond within the timeout period.
Together with EventID: 1215, Source: Storage Agents.
Fibre Channel Controller Status Change. The Fibre Channel Controller in Slot 3 has a new status of 2.
(Host controller status values: 1=other, 2=ok, 3=failed, 4=shutdown, 5=connectionDegraded, 6=connectionFailed)
[SNMP TRAP: 16028 in CPQFCA.MIB]
Multiple Windows systems, using B-series switch connected to EVA5K Using latest EVA kit and Secure Path.
How to get rid of those messages = fix problem if exists.

Sound like the HBA is possibly flaking out.
Event id:1215
Symptom: Fibre Channel Host Controller Status Change. This trap signifies that the agent has detected a change in the status of a Fibre Channel Host Controller. The variable cpqFcaHostCntlrStatus indicates the current controller status.
User Action: If the controller status is failed, replace the controller.
