Raid5 failure

We have lost again the complete Raid 5 logical drive. This is the second time that this problem append.
We have this configuration :
DL380 with 6 disks :
Disk 0 and 1 : Logical 1 : RAID 1 : System
Disk 2, 3, 4, 5 : Logical 2 : RAID5 : Datas

Last week, the disk drive ID4 of the Raid5 logical drive fail. We were expecting a continuing activity (with server charge, of course). At this time, access to the D Drive (logical 1) was mark as unknow format when we try to access.
Ejecting the faulty disk and replacing it by the spare give nothing and no rebuild was made. We have restart the server and eject and then reinsert the drive and then the logical drive start the rebuild. This does not correct the problem and backup tape have been restored.

We are affraid about this incident because we are wondering about the other servers where it can append. We have to be confidant on our smart array integrated controllers. RAID 5 is a fault tolerance !

The carepaq incident was unsuccessfull. Nobody was able to understand what could have append to the smart array.

Are you using the Array Diagnostic Utility to tell you the status of the rebuild? The ADU might also tell you of any error conditions.

Has someone in support looked at an ADU report from the server?
