SolutionHave you installed the latest SCSI patches.
Check whether the following patches are installed.
# PHKL_30510 1.0 SCSI IO Cumulative Patch
# PHKL_30511 1.0 SCSI Ultra160 Cumulative Patch
# PHKL_23666 1.0 SCSI IO Subsystem Cumulative Patch
Use the command:-
#swlist -l fileset | grep -i scsi
Also,see this thread. There is a similar problem. This will give you some imputs on solving the problem.

Yes, Syam is right!
The error is SCSI error. You need install some patch file that mention in the thread which Syam give you.

When you are taking next down time for server. Try to make it in 1-2 weeks maximum.
Hope you are taking cold backups of servers every week. During that down time you can do this patch application too..Down load the patches make single depot and be ready. Then it can be done in no time and only single reboot required.

What about the application is running on your system?
Oracle database?
Anyway, to install the patch file, no need to take a cold backup.
Just install the pathc file and then reboot the server.
Only the patch file with PHKL_XXXXX prefix is require reboot the server...

Cold backup is a regular activity at my place. We do it every week taking few hours downtime for our globally accessed production servers. I thought you all also doing it regularly.

When are u normally getting downtime for the servers? I feel you need to completed the patch updation as soon as possible. Apart from the mentioned just confirm that you are updated the the server with the latest available patch bundles.
