We are having QLogic HBA installed in 1 of our Proliant box, how we check the details and model of HBA in Windows 2003 Server?

Install a product called SanSurfer. You can download from HP or Qlogic page.

Do you not have HP Management Agents/Storage Agents installed on the server? The management agent provides all the info that you need for the FCAs under "External Storage Connections" on the management agent home page accessible on https://servername:2381
