Can anybody please send me the list of all the items to be covered during transition for HP-UX system administartion and Oracle database administration for taking handover from Vendor.

You mean UAT?
If so for HP-UX, they will provide you the following checklists
1. Power up test
2. Power redundancy test
3. CPU verification
4. Memory verification
5. Hard disk functionality
6. tape drive verification
7. DVD-ROM verification
8. Network functionality
9. OS bootup sequence
10. administrator login
11. NIC configuration
12. network testing
13. hostname resolution
14. kernel parameters
15. vlume groups and LVM
16. swap partitions
17. softwares
18. patches etc.etc. as per your requirements if added anything
for Oracle functionality you can ask your DBA for their UAT checklists.

- Backup/Recovery
- Patch Installation
- Monitoring and definition of bounds (how to escalate out-of-bound activities)

The plan very much depends on what oracle products will be installed and how they will be used.
To date, you have not even identified the type of system you are using, the back end OS or the Oracle database.
There is no cut and paste plan that can be delivered to you.
Your application vendors may not support every OS version choice you have.
So here is how you start, a little survey:
1) What Applications do you want to run?
2) What are the Oracle requirements for the applications you want to run?
3) What is the uptime requirement for these applications?
4) What high availability infrastructure are you willing to pay for to meet the requirement in question 3.
5) What OS version has the latest support and longest support life cycle based on earlier choices.
6) OS and hardware choice phase.
7) Write a plan based on choices listed before.
