Just shutdown the instance.
try starting it again with the right instance name and try re-creating it.There may be some oracle de-funct process running,may be ....

Sorry for lot of replies.
I was just finding the different causes.
Just even try this below
cd $ORACLE_HOME/rdbms/lib
make -f ins_rdbms.mk install
cd $ORACLE_HOME/bin
./relink all
shared_pool_size too small
also check the permissions
The oracle executable in the $ORACLE_HOME/bin directory should have the permissions:
-rwsr-sr-x 1 oracle dba 34049236 Oct 11 17:17 oracle
