Registering for HP support

We have serveral clients who purchased VMware Acceleration Kits through HP which include 1 year 9 to 5 support. While trying to log a call with HP support I was told that we needed to register to recieve this support somehow, though nobody seem to know how.

I have spoken to serveral people at HP who were unable to assist, I did come across an excel worksheet to fill in, the instructions said email it to the address at the bottom, but there was no email address anywhere on the form.

Can anyone tell me how to go about this, we do not have an account manager at HP, we just miss out on qualifing for partner status and the company we purchased it from were not able to help. I have the license keys provided by HP but I don't have the leaflet it was stuck on.
