I have tried to add the user in to Relay in the access file, I tried OK even. Not luck

It will except his name as a sender, but it will not send anything outside our domain for him. He can send to our local users just fine.

Well, you need something in sendmail.cf to utilize relaying features. Do you have the mc file that was used to build it??

On an 8.12.8 sendmail server, I have
/etc/mail/relay-domains

and
# Access list database (for spam stomping)
Kaccess hash -T -o /etc/mail/access.db
