check the log files
/var/opt/ignite/recovery/latest/recovery.log
/var/opt/ignite/recovery/latest/flist
/var/opt/ignite/local/install.log
Recovery log -- inputs what was on the screen when tape ws created
Flist log -- inputs what was backed on the tape
Install log --inputs what happened when system was restored.

I suggest you should try using new media cartridge & if it still gives you IO error then you might need to replace your tape drive. IO error suggests issue with the tape drive normally.

My problem have been solved. After I turned off my server and tape drive and restart them, then I execute make_tape_recovery -Av. That's OK!

is your aim to create a bootable tape by make_tape_recovery including vg00?
Consider that the option -A on make_tape_recovery doens't have the same goal which had the obsolete make_recovery. The related option on make_tape_recovery is -x inc_entire=vg00.
Definitely what was
make_recovery -Av
now corresponds exactly to
make_tape_recovery -v -x inc_entire=vg00
