If this is a HDD you can check for read and write errors in cstm, by running the following commands.
#cstm
cstm>map <== check the hardware path of the device which is reporting lbolt, make note of the device number...
cstm>sel dev
cstm>info
cstm>il <== this will give you the complete information about read and write errors...
Any read & write errors replace the disk immediately.

SolutionLbolt errors are actually from c0t4d0 which is a tape drive connected to this system C5683A (suspect it should be a C5300 tape array a DAT 40 DDS4 module), suggestion would be to check ..
1. If there was any backup process running when this error reported.
2. Check if cable connected from server to drive
3. Check for stape and scsi patch level
4. If still issue persists get the drive replaced by logging a support ticket with HP.

The device format is: MMBBTLFF
Where : MM = Major number
BB = Bus number
T = Target
L = LUN
FF = Flags
ls -l /dev/rdsk/*|grep 004002
yields the cXtYdZ of the disk device.

Help me out with this, I don't get your cstm procedure or your conclusion. Albeit, tape drive's are a very good guess, and c0t4d0 is a very good guess, yet I don't see any backup for this. By your formula I should be looking for c0t4d2 but this doesn't appear.
When I use cstm I don't use inforlog as it is not built for tracking errors the way logtool is.
