powerfailed message and LV unavailable

Can someone tell me that why (LVM: vg[8]: pvnum=5 (dev_t=0x1f093400) is POWERFAILED) is going into POWERFAILED state there are no scsi I/O errors reported before POWERFAILED shows up. I have even tried chaging the timeout value to 90 sec using pvchange -t /dev/dsk/ and it has not helped. The LV state for this device shows as unavailable. We had lost access to the storage device when the problem showed up but now storage is all working and we have rebooted the host (servicegurard cluster) and did ioscan and vgscan -a and still is not helping. Doing dd (reads only) we see I/O from the host to the storage. Can't understand why showing up the powerfailed messages, OS is HPUX 11i.

what is the output from

# ioscan -fnC disk

Well all the disk shows up as claimed state.Why do you get a POWERFAILED message? I saw some forum comments saying that when you loose access to a device but in our case we see reads happening and if there are any scsi write failure they should show up as write errors rather then powerfailed message.

Figured out the problem exists on the storage side and have resolved the issue.
