Because you did not specify OS version or any other context (syslog? boot message? something else?), I'll have to guess.

The message does not look like HP-UX standard messages: I would have expected the disk to be identified using either a device name (like /dev/dsk/c1t2d0 for example) or a hardware path (like 0/0/1/1.0 for example). Or are you showing us just a part of the message?

My guess is that the self-test results of a disk with SCSI ID #3 on SCSI channel #1 of an unknown SCSI controller are indicating that the disk might fail soon.

You should now find the correct disk and make sure you have up-to-date backups of the data on that disk.

If the disk is mirrored or a part of a RAID5 set and can be hot-swapped, it's not a big deal. Just make sure you know the correct hot-swap procedure and have new disks easily available.
