All your EVA LUNs are still presented in a wrong mode (likely windows or default instead of hpux) - check this!
Let's count the pathes for LUN 1:
disk 487 0/12/0/0.1.0.0.0.0.1 sdisk CLAIMED DEVICE HP HSV200
/dev/dsk/c15t0d1 /dev/rdsk/c15t0d1
disk 130 0/12/0/0.1.2.0.0.0.1 sdisk CLAIMED DEVICE HP HSV200
/dev/dsk/c9t0d1 /dev/rdsk/c9t0d1
disk 606 0/12/0/0.1.5.0.0.0.1 sdisk CLAIMED DEVICE HP HSV200
/dev/dsk/c17t0d1 /dev/rdsk/c17t0d1
and this is how a wrong presented LUN looks like:
disk 599 0/12/0/0.1.0.255.0.0.1 sdisk CLAIMED DEVICE HP HSV200
/dev/dsk/c16t0d1 /dev/rdsk/c16t0d1
This will disappear if you configure the correct mode.
BTW, the EVA 4100 has 4 host ports - did you plug 3 of them into the same switch?
