was it after adding new memory modules?

Which system in this one ? Is it a RP54xx ?
The reason for this seems that you have not populated the SIMMs in proper order as mentioned in the cover of the box itself. I would suggest you try to boot the system with minimal required memory installed first to eliminate other possibilities.
Also take some time to assign points to previous responses to your questions as it is your only contribution towards this forum. If you have assigned points for all your earlier responses(which is displayed in your profile), the responses also will be quick.

From your recent logs it looks like primary boot device is not responding(probably hard disk is failed) Try to boot from mirror copy if you know the path or if the path is set as alternate.
Secondly, you can search for possible boot devices at ISL prompt.

1.if your memory is not inserted properly.
2.if you add new memory to your server.
3.if you should add the large capcity memory first, then the small capcity memory.
4.if your server need insert the same capcity
memory into the slot number (4a,4b,4c,4d,4e,4f).

Which Model of Server is this.
I had faced similar problem before, as rightly mentioned by others here, you can reseat the memory sticks, also check the order (0a,0b,1a,1b) as mentioned on the Server
regarding the second error, reseat the harddisk
also you can try and change the boot order ( you can get a list of boot devices with search from Main menu (BCH Menu)
