hi eduardo,

this is clearly disk problem.

Replace the disk and from boot handler, boot the tape which contain your make_tape_recovery backup.

1) search for tape in boot handler. It may take a while.
bch> sea

2) Look for device which are tagged as sequential device. Insert the tape and boot it.
bch > boot p0

3) It should be able to recover straight away as there are no hardware difference. The only diff is you replace the dead disk.

hi,

the fault codes starting with B are all OS dump codes.

the system is starting to load then crash for some reason or other.

likely hood its either the disk has got some bad sectors and cannot be read, but dont rule out the scsi IO board aswell, as you have lots of scsi errors aswell as lbolts.

looks like a new disk and re-install or recovery

Thnks for your replies....Ill do the system recovery when i go back to our client....this will surely help me solve our problem
