just a few questions: what CD did you use? Did you terminate the SCSI bus? This alert doesn't give enough information.

Check the console path (for GSP version B.XX.XX) , see the url below :

for GSP version A.XX.XX :

From GSP for other log around the time when you try to install the OS.
Also check the GSP firmware, the latest one for revision B GSP firmware is C.02.14.

1. Why did you chose to reinstall OS?
2. There is a possibility of hardware failure in the system. Check GSP logs
