We recently found a couple of servers attention led flashing, when I checked the chasis log, it showed the following, any idea what that means?

The Bulk Power Supplies for the cabinet are illegally configured
Description: Through failures or reconfiguration, the BPS for the cabinet named are illegally configured. There must be a BPS connected to each phase of the power. Phase 1 feeds BPS slots 0 & 1, phase 2 feeds slots 2 & 3, and phase 3 feeds 4 & 5. There must be a BPS connected to each phase. If 4 BPS are installed in a cabinet in slots 0 - 3 and 4 & 5 were empty, this would be an illegal configuration. They should be installed in 0,1,2,and 4 or 0,1,3,and 5 or some combination thereof.

Solution
Start reading from the bottom. The first thing you see is that AC was deleted (lost power to the box) at 12:57:14, then added (restored power) and lost again within 60 cycles, then added at 12:57:17 and finally lost again at 12:57:18. You have/had a power problem. I would say that one phase into your box was having problems. (I've seen this happen when a squirrel arcs itself across the power lines, frying the squirrel and shutting down power in the attached building.)
Remove the front panel from your server and check the lights on the BPS's. They should all be on solid and green. As stated above, check their positioning.
From the MP run CM and then PWRGRD, if you have all the power cords plugged in you should have BOTH or A and B.
If either of these are not true, investigate it further.
