Your sysrev output does not match the rp8400, but the rp8420!
I can see an inconsistency also:
Cell 2
PDHC : A.003.030
Pri SFW : 22.002 (PA)
Sec SFW : 24.001 (PA)
But on all other cells Pri ans Sec is like that:
Cell 1
PDHC : A.003.030
Pri SFW : 24.001 (PA)
Sec SFW : 22.002 (PA)
Regarding your initial issue:
This is a known problem fixed in the newer firmware release.

Solutionyour title is a little bit mesleading. I guess that you have a rp8420, not a rp8400 ;-)
You are running on firmware bundle 4.0.
The firmware bundle 4.1 with MP firmware A.008.006 contains the following fix (beside some other):
"Previously, when the MP:CM> PS command was used to display the cabinet state of the SEU, Bulk Power Supply 1 would not indicate "Power Status OK". The PS command now correctly displays the BPS state."

=> you have no hardware problem. You can safely ignore the wrong status and wait until the next planned downtime to update the firmware.
