I have an MA8000 subsystem with hsg80 controllers running ACS 8.6P-10. I am getting errors as shown in the attached document when I try add a disk on any target on port 3 and 4. Please help.

Solution
There is a problem with the disk drives or the slots.
Instance code 03F30064 means that the controller port had to be reset to 'clear a bad condition'.
Instance code 03D04002 means that the controller was unable to talk to the device - a 'test unit read' or 'read capacity' command failed.

Thanks Uwe. I used to think that the disk drives were faulty and I bought brand new disks, but I'm still getting the same problem. Is there anything I can do.

A colleague of mine did put a new disk drive into a storage array and it blocked the whole bus!
You say that the disk drive doesn't work on any free position. I would try the following:
- use the spare disk (DISK40000) to test out these slots
- use the disk position DISK40000 to test the new disk
