EVA Disk disconnecting
Hi,
I've got a BL685 G6 Server running Windows Server 2008 Enterprise R2 with Service Pack 1. It has MPIO V4 installed. The servers has a QMH2462 FC card installed and is connected to an EVA8100.
We are having issues with 1 EVA disk which disconnects every 7 days or so, Can see all other disks in the Management Console apart from this 1 disk. The errors in the Event Log are below:
All paths have failed. \Device\MPIODisk11 will be removed.
A fail-over on \Device\MPIODisk11 occurred.
Any ideas

I just came across this kind of issue at customer site. This is caused by issues in Windows 2008 mpio.sys. Please check the following KB articles...
