Full Fault-Tolerance for MSA 2000i G2?

I haven't been able to find clear guidance or even an example configuration that shows how to achieve four redundant paths to a LUN on an MSA 2000i with redundant controllers. The four desired paths would be:

1. Host interface 1 on controller A.
1. Host interface 2 on controller A.
1. Host interface 1 on controller B.
1. Host interface 2 on controller B.

The Windows 2008 MPIO configuration seems to recognize only two paths - one per for each iSCSI NIC on the server.

Any suggestions on explicitly enabling all four paths?
