MPIO windows 2008 R2 and EVA 6100
I have a Windows 2008 R2 connected to EVA6100 via 2 FC switches. (4 possible ways to see LUNs)
My problem is I can see 4 disks per LUN presented from the EVA.

I currently have one (of the 4 visible) LUNs assigned (windows is using it)
What do you think is going on here?

What XCS version is the EVA running?

Also check that you have defined the host on Command View with the correct Host OS mode (WindowsLH, not just Windows).

I cannot find anywhere that the version I have is supported for R2. Could this be the issue?
I believe I have to upgrade to EVA 8.0.2 then to "xcs 6220" in 2 steps.
Any other help?

upgrade CV to latest 9.2, then upgrade firmware to v6220.
come back after this.
We do not use win2008 R2, but have no problem with even older Eva6000 and win2008 prior to R2 (R2 not tried yet)

You are already at CV EVA 8.0.2. You "could" upgrade to 9.2 just for support reasons... but you probably do not need to in order to get things running.
The only steps you need to do would be to:
1. Update the Disk Drive Firmware
2. Update the EVA to XCS 6.220
