the answer from HP-Support:

The workarround for this moment would be: delete all and start clean.
Don't use online VDISK intialization.
Make a configuration planning this to avoid to creating and delete items in one line.
To avoid this controller behavior we also working to get a fix at the controller code level.
At this moment no other workarround than recreation is available.

P.S. we replaced the controller and the chassis of our MSA and it did not solve the issue, the only thing that sorted it was enabling the Independent Cache Performance Mode.

I was suspecting that the MSA was thinking he had two controllers in stead of one. But reverting to the old firmware did not help.
Well...I enabled Independent Cache Performance mode and will see what happens.

The overall size of the vdisk should not make any difference, i am not aware of any limits on size of the vdisk.
I am currently unable to connect into our clients MSA but when i do get access i will have a look for you and see if i can see anywhere where you can set it.
Failing that i believe we have a test MSA in our office which i might be able to play with.
Is the MSA still failing (as per your initial post)? I am curious to see if the fix i suggested worked for you as well.
