Resolution for the above issues:

Identify the malfunctioning cable or SFP specified in the issue text. Remove and re-insert the cable/SFP and verify that the port Status LED is solid green and that the amber Fault LED is not illuminated. If re-seating the cable/SFP does not resolve the issue, replace it.

Is it just because I removed 2 SFP+ from each controller?

If the only issue is missing SFPs, and you're okay continuing with the update, then you can re-run the update and ignore the warnings. Use the Re-Run button at the bottom of the View Details dialog to do so.
