I have the following ERRORs after execute make_recovery_tape for a vg00 bakcup:

Can U execute "ioscan -fnC tape" command & check Ur tape is "CLAIMED DEVICE HP C7438A" or "UNCLAIMED DEVICE HP C7438A" if it is "CLAIMED" then U can execute below commands:-
"make_tape_recovery -x inc_entire=vg00 -v -a /dev/rmt/2mn"
or if Ur tape status "UNCLAIMED" then Un should follow below process:-
1. Power off Ur tape library
2. Check the SCSI cable all are properly connect or not
3. Power up the tape library
4. ioscan
5. insf -e
6. ioscan -fnC tape
Then follow make_tape_recovery process.

To make an Ignite tape recovery image requires a no-rewind tape device, hence the 'mn' notation with the unit number. This is because Ignite writes a bootable LIF image on the tape; does NOT rewind the tape; and then writes a 'pax' (tar) archive following the LIF (bootable) image.
If you were to rewind the tape between writing the LIF and the Ignite archive, you would be left WITHOUT the LIF and have an unbootable, useless recovery tape.
The manpages for 'mt(7)' describe tape devices.
Once you have made a successful Ignite recovery tape you can also recover a single file from it:
# mt -f /dev/rmt/2mn fsf 1
# tar -xvf /dev/rmt/2m filetorecover
This again shows the use of a no-rewind tape device to leave the tape head positioned AFTER the LIF and at the beginning of the 'pax' archive --- exactly like you would extract any 'tar' or 'pax' archive.

Besides the details that Bill gave, it wasn't working before because Ignite said it wanted a no rewind device. It let you know before you were hosed.
