SolutionMay I know the complete command that you've been used for make_recovery ?
If you using make_recovery command, need your help to attach the log file below :
/var/opt/ignite/logs/makrec.logXX
PS : please attach the latest one (check the timestamp)
If you using make_tape_recovery command, need your help to attach the log file below :
/var/opt/ignite/recovery/latest/recovery.log
With these information, we can try to find out the problem and how to solve it.

Error 1 - using make_recovery, which is obsolete.
It is helpful to know the OS version, version if Ignite and command syntax used to further assist on your issue.
swlist -l product | grep -i ignite
uname -a
The error could be caused by an old version of VXVM, or LVM inconsistencies.

Thanks for the info.
Looks like something wrong with the disk '/dev/dsk/c2t2d0' and this cause the make_recovery failed. See the error below from makrec.log1 :
lvdisplay: Warning: couldn't query physical volume "/dev/dsk/c2t2d0":
The specified path does not correspond to physical volume attached to
this volume group
lvdisplay: Warning: couldn't query all of the physical volumes.
lssf: Must specify a special_file
usage: lssf special_file ...
save_config: error - unknown disk type for , not SCSI or HPFL
save_config: error - cannot determine root disk
So need you to check the particular disk with the command below :
# lvlnboot -v
# ioscan -nfCdisk
# pvdisplay /dev/dsk/c2t2d0
# dd if=/dev/dsk/c2t2d0 of=/dev/null bs=1024k
If the particular disk (/dev/dsk/c2t2d0) giving the NO_HW status (on ioscan), I/O error (on dd command), OR any other error; you need to replace this harddisk first.
FYI, you have the completed one on Wed Mar 2 09:28:35 2005.
You can attach the output of the commands above, so I can help you further to solve this.
If this problem solve, I refer you to update your Ignite/UX software so you can have the latest Ignite/UX software on your system (less bugs). The software can be found at this url below :

Yes, the particular disk (c2t2d0) is missing from your system.
It looks the disk belong to vg00, check with the command below :
# strings /etc/lvmtab
If you have HP support, please call them immediate. You need to replace this harddisk.
You can restore the OS with the tape from Wed Mar 2 09:28:35 2005 first and restore again from the latest available backup.
