Pls post the map output and ls -r from fs1:

Seems there are two disks visible to this system, both are the first partition PART1 on another disk on another controller.

I'm not sure if anyone swapped the drives, so I did a ls -r on fs0 and fs1 again. I also added the map output into a zip archive. Thanks.

It doesn't appear to let me attach multiple files, so I'll just attach them one at a time. This is ls -r for fs0.

Very interesting. From the map output you see that two Pun's are assigned to the same drive.

fs0 : Acpi(HPQ0002,PNP0A08,400)/Pci(0|0)/Pci(0|0)/Scsi(Pun0,Lun2)/HD(Part1,Sig336FF44F-9681-4646-B022-AB65E7EF36DA)
fs1 : Acpi(HPQ0002,PNP0A08,400)/Pci(0|0)/Pci(0|0)/Scsi(Pun6,Lun0)/HD(Part1,Sig336FF44F-9681-4646-B022-AB65E7EF36DA)

Both Pun instance #'s GUID number is the same.

Start the SAS raid config utility from the EFI shell and see what that reports for devices and topology. Start this with >drvcfg -s

The drives are attached to a P400 Smart Array controller, not the built in SAS controller.

OK, then start the offline config utility for the card. Reset the server and watch the console. Your given a few seconds during boot up to enter the utility (by pressing F8 I think).

Im not sure what you will see, but perhaps something will stand out for you.

I've looked at the P400 setup more times than I can remember. There's nothing odd that stands out. I've compared it to the other rx2660s that we have and it looks exactly the same.
