Re: IO Type : MPT Domain Validation IO has timed-out
The logs are telling you the source of the problem:
Dec 24 16:21:32 ECCDEV vmunix: SCSI Ultra320 0/3/1/1 instance 2: IO Type : MPT Domain Validation IO has timed-out. Target ID: 1, LUN ID: 0. Inquiry Command - CDB: 12 00 00 00 40 00
A SCSI card at location 0/3/1/1 is reporting a problem from an attached device with a target ID of 1. Quite possibly you have a problematic disk drive. For the rx3600, path 0/3/1 is PCI slot 4.
The STM info you also included supports this by reporting an 'unknown' state for this device:
18 0/3/1/0 MPT SCSI Adapter (HP_A717 Information Successful
19 0/3/1/1 MPT SCSI Adapter (HP_A717 Information Successful
20 0/3/1/1.1.0 Unknown ()
I would look at replacing this disk or whatever it is.

Re: IO Type : MPT Domain Validation IO has timed-out
Dont be so sure. A failing drive can produce errors without actually being "hard failed".
To view the card details run ioscan as follows:
# ioscan -fnk
Look for the path 0/3/1/1 and then look for the assigned device driver instance. For example /dev/mpt1
Run the command mptutil /dev/mpt1
This will give information about the controller.
