The blk device is listed, so the system "see" the device. Strange. It is the correct DVD 1? Try to power cycle the server and check the mapping again.

Yes, that what i said, is very strange, i tried with another media installation disk, i reboot the server, power off and map again with another disk but the server does see the media.
I have not tried with another DVD unit, but i do not think DVD unit problem becouse the server see the dvd unit from single user and efi shell.

AFAIR there was a possible problem with the drive, but I'm not sure.
How about firmware?
Current version is
MP:CM> sysrev
MP FW : E.03.32
BMC FW : 01.30
EFI FW : 03.20
System FW : 03.33
