This is a part from your output:

/dev/dsk/c0t2d0s2 (0/0/2/0.2.0) -- Boot Disk
/dev/dsk/c0t0d0 (0/0/2/0.0.0) -- Boot Disk

You can see, there is a device "c0t2d0s2" and a "c0t0d0". First one is working, first one has *partitions". You will see 3 different partitions for this disk in ioscan:

c0t2d0s1 - EFI Boot partition
c0t2d0s2 - OS
c0t2d0s3 - system utilities

Your second disk has only 1 "partition" for OS, boot area is missing! So this disk cannot boot!

You need to follow the given procedure (see link above) to mirror a boot disk in itanium servers, create disk partitions (idisk) and so on...
always use the ...s2 device for LVM related commands!
