We have changed the internal disk twice and made it in both slots get the same error message. Also try to install from an Ignite tape.
Please anyone has any suggestions to help us to install HP-UX.

Hi,
I've reviewed a similar old case, indeed with same OS, HW model and affect at same disk address (hex=1f03f000. Device=c3t15d0)
However, this case is different because occurs while installing the operating system, making difficult any test by commands.

Also, according to your comments, it not seems a power failure associated with the disk by itself, can be any element attached (power socket, flatwire, etc). Maybe a controller firmware update could be required. Is your disk newest (burda) than server?
As you see, recommendations are contact with HP Hardware Support.
