If one disk on the array died and re-built from hotspare, why you should get a PVlink failure?
PVLink failure will occure when there is a connectivity loss. Local FC, switch or the link at storage end can cause this.
I suspect storage guys .....:(

From the logs, I see that you actually lost few PVs .....pls check with SAN team why it happend
