ioscan shows CLAIMED but do not appear in vgdisplay

Hi gurus,

We have 4 disks under vg00 but disk c4t8d0 is not appear when we execute vgdisplay command. From the ioscan, it shows CLAIMED. Diskinfo command shows size 0 KBytes. It's alternate boot disk. We plan to change the disk. Do u mind to share with us steps that we need to take to replace the disk?

->  Replace disk

# ioscan -fnCdisk

# pvcreate -fB /dev/rdsk/c4t8d0

# mkboot /dev/dsk/c4t8d0

# mkboot -a "hpux -lq" /dev/dsk/c4t8d0

# lifls /dev/dsk/c4t8d0

# lifcp /dev/dsk/c4t8d0:AUTO -

# vgcfgrestore -n vg00 /dev/rdsk/c4t8d0

# vgdisplay -v vg00

# vgchange -a y vg00

# vgdisplay -v /dev/vg00 | grep -i stale | wc -l
