replacing a failed mirror root disk - hpux 11iv3

am replacing a failed mirrored root disk, os is 11.31. am doing an "idisk -R " as i hd used this disk in another system.

# idisk -R /dev/rdisk/disk4
idisk version: 1.44
********************** WARNING ***********************
If you continue you will destroy all partition data on this disk.
Do you wish to continue(yes/no)? yes
# idisk -wf /hp/dk /dev/rdisk/disk4
idisk version: 1.44
********************** WARNING ***********************
If you continue you may destroy all data on this disk.
Do you wish to continue(yes/no)? yes
idisk: Write of requested data failed(11)
idisk: Write of EFI data failed.

What server model do you have and where is the disk connected? Is it a SAS disk?

Did you perform steps 4-7?

4. Notify the mass storage subsystem that the disk has been replaced.

...

7. Assign the old instance number to the replacement disk.

tht ws a superdome.
it ws sending lots of scsi resets to the diagnostic daemon and the problem identified to be as a loose connection.
reseated the cable, POFF/PON the DS2120 and it went thro.
i hv anothr servr reporting this problm when i reach this step of mirroring the disk..
rx8640, hpux11.31, DS2120, scsi disks

# mkboot -e -l /dev/rdisk/disk3

Could not copy /usr/lib/efi/EFI/HPUX/hpux.efi file to EFI/HPUX/hpux.efi

ERROR: /usr/sbin/efi_cp returned 26
