Do a strings at /etc/lvmtab and then do an ioscan to see those device files appear as claimed

I too received this error a day ago and the lvols were not mounting, throwing d same error.
According to me this occured b'cos u had RESTARTED the system which might have abruptly caused a problem with d LUNS.
Try to SHUTDOWN the system and then start I hope tht would solve ur problem.... cos this is how it worked for me...
I m jst trying 2 find out the cause of this error :)

Are you sure you can see the lun after rebooting?
try with:
#diskinfo /dev/rdsk/c5t0d0
also try recreating the your lvmtab with lvmtab, and have a look if the disk is there.
if you are in 11.31, why don't you use agile view ?

It looks there are some issue with LUNS availablity or I/O error with Luns.
How many luns are there in vg02?
Could you provide the following?
#ioscan -fnC disk
#strings /etc/lvmtab
#vgdisplay -v vg02
#pvdisplay -v /dev/dsk/c5t0d0
#diskinfo /dev/rdsk/c5t0d0
#dd if=/dev/rdsk/c5t0d0 of=/dev/null bs=1024k count=25

Issue resolved after modification being done on clarion.
HP-UX 11.31 volume group on an EMC Clariion CX320, the following steps in Navisphere need to be done.
The Clariion Flare code must be at level 26.
The host must be defined as Clariion open .
The fail-over type must be 4 . (This makes the array look like it is active/active .)
