vmunix: ct_query failed. hw_path = 0/10/0/0

Has anyone see this error before? The path is to our HP Tachyon XL2 Fibre Channel Mass Storage Adapter on an rp5400 running 11.00. I'm getting the same error on both adapters.

What do you get from the above ioscan ?

Looks like the fibre channel cards are connected to the switch but no devices were
configured i.e zoning has not been done for any devices to be seen by this card.

Were they working before ?
Learn What to do ,How to do and more importantly When to do ?
