perl, html form -> html stat file

hi all,

As you know I'm not a great perl monger and as I know, lots of you are ;)

I'm overwhelmed by perl resource searches, so perhaps you can help:

How can I get the attached html form results saved to file and reported all pretty !

I'm sure this kinda stuff has been done before.. I just can't find it!

Lots of points awarded!

ie:

course: Bills cooking class
...
..
.
date: 1/1/2002
...
..
.
Q42 : results
40% Excellent
50% Fair
10% Poor

Q43 : results
90% Excellent
10% Poor

Plain output, or do you also want the radio buttons, drop-down boxes and other decorations?

If you only want a reformat, I'm willing to give it a shot, with interactivity, I'm not. I don't do HTML/CGI.

drop me (merijn at procura dot nl) the input data if you opt for txt2html only
