HP B-series 8/12c SAN Switch + MSA2012fc - missing path

Hi,

I have a c7000 enclosure with two HP B-series 8/12c SAN Switches. My MSA2012fc has dual controller they are connected to both switches (Controller 1 port A to SW1, port B to SW2, same for the controller 2).

I have blades with vmware esx and also blades with win2008 installed. Both config can only see 2 paths to the volumes, one on each HBA.

What is wrong? They should see 4 paths.
