Need a replacement ITILv3 Certification form

Took and passed the ITILv3 exam. Got a Certification of Completion for HP training conducted by P Wilmot in Dallas on 1/25/11;  got the cert in the mail.  Moved my office in the house,   lost the certificate.  How do I get a replacement. I can PDF and send the completion form (or perhaps you have me on file there).  Who do I contact?  Thanks,
