Memory Leak Troubleshooting W2K3

Hi,
Every fourth week wee neeed to reboot a windows server on the basis of to little memory. Huge amount of processes are running on the same server, and I don't know which process that casue the problem. I also know that it's possible to do some investiagations regarding Private Bytes, Working set, Available Memory ++ in co-operation with perfmon. I have tried to run this procedure on specific processess, but so far without any results. Hope someof can give me usefull hints/tools/SW , howto find the process that cause the leakage.

get a copy of procexp from the microsoft sysinternals page, configure it to show all memory options, then run it continuously on the console and you can track processes memory usage.
