are there any changes in the environment, like upgrades? You check whether are there any files from prev. version (if an upgrade happened).
Try
$ dir/date amds$system:amds$rmcp.exe
$ dir/date sys$loadable_images:sys$rmdriver.exe

Have you installed a later version? If so, you will
need to reboot to get the latest driver.
I also recall seeing something about early versions
putting things in the wrong place and not being cleaned
up properly when upgrading (but am unable to locate
that information at present)
You don't mention your platform, VMS version, AMDS version.
Providing this information usually makes it easier to
diagnose problems.

Solution$SET DEF AMDS$SYSTEM
$DIR AMDS$RMCP.EXE
$! Delete the old version of AMDS$RMCP.EXE
$DEL dir_spec:AMDS$RMCP.EXE;1
$! You still should have the newer version
$! Unfortunately a reboot is necessary to complete this.

pay attention to all files which reside in SYS$COMMON:[AMDS] !!!
There are also the old version of AMDS$DRIVER_ACCESS.DAT, AMDS$LOGICALS.COM etc. which can make life harder!
OR / AND use AMDS$SYSTEM instead of SYS$MANAGER (like us!)!
