Have you tried re-loading the console firmware (and adapter firmware) here?

Solutionas far as I can recall, the CIPCA-BA is a 5.0V PCI card, so you would need to place it in two adjacent 5.0V PCI slots rather than the 3.3V slots.
See fig 4-16 in the attached link for slot specs on the ES45 model 2B

What will we connected to the CIPCA?
A dual path Cipca will still be limited to 140mb/second roughly single scsi-2 speeds.
And the controllers and drives behind them are not too fast either by todays standards. ( The fibre channel connected EMC stuff )
I'm guessing you just want to connect to some legacy storage which still has to be maintained for other system needs.
From a performance, footprint and energy use perspective you should probably ditch the whole lot, but that may not be realistic.

The 2 Gb Fibre Channel SAN gear is looking a little slow these days.
The CI 2 x 70 Mb gear here is way, way slow.
Some legacy hardware configuration being retired or scrubbed or such, may I presume? It might be easier to move the disk shelves over than to fuss with a CIPCA and CI cabling, if that is the case.

I'm going to agree with Duncan. You will have to use the four 5volt slots in the model 2B. Specifically PCI slots 3,6,9,10. Its been a long time since I've used a CIPCA, but off the top of my head I would think the intermodule ribbon cables would be long enough to span over slots 4 and 5.
