So I need to change the substitute_domain to blah.com so it matches our internal email addresses and the Zone to the same then get the exchange guys to create a mailbox with the vms_username@blah.com so it can be resolved?

Yes, the VMS setup will set the correct sender parameters for the mail domain part.

The user from address You have to agree with the mail/exchange manager: either You have mailboxes there for all VMS users in question,
or put all VMS users in a forwarding or alias list on the incoming mail gateway.
(I don't know how to do tyhat on an exchange server, I only know Linux mail setup).

and, third possibility, You can allow users on VMS to set their FROM: address to their exchange server name (if any) via
DEFINE TCPIP$SMTP_FROM "user@ourmaildomain"

Logical
TCPIP$SMTP_PROHIBIT_USER_HEADERS
must not result in TRUE to let users set their own From:
