I am getting these errors, Please help us.

More info would be helpful.

What do the messages file, syslog logs and binary.errlog say ? Has dsk1 had a problem perhaps ?

hi abbas,
execute the following:

# volstat -sff vol12

if you see read/write errors values are more than 0, its time for you to change disk.

Really appreciate if you could assign somepoints if this answer ur question.

i have RAID 5, if my disk is faulty then no red light on disk, how i will know that which disk is faulty. please help

Your var/adm/messages attachment suggest there is an Smart Array 5300A attached.
I'm sure that has the RAID-5.
You will have to ask it what to do next, using the tool for that which can be found on the CD's:
CPQACUXE13070.TAR ACU-XE Configuration Utility for Tru64 UNIX

Just google for "Smart Array 5300A" +tru64.
Start with "Smart Array 5300A Controller Option Kit Read Me First"
