Current OS Version: V5.1 (Rev. 732)
Upgrade to: V5.1B
Our upgrade Steps:
1. Firmware upgrade: succeeded.
2. Upgrade from the CD labeled "HP Tru64 UNIX Operating System V5.1B": Failed.
The machine can NOT recognize the CD. We also tried mounting it in the OS, failed too.
At the same time, we can mount this CD on other machines without any problems.
3. So we made a tar ball of the contents in the CD on another server and FTP it to the DS20E, untar it and tried to upgrade from local disk, but failed again. Any detail information please see the upgrade.log attached.
4. After the unsuccessfully upgrade, the system can not boot up now. Such error occurs during system bootup:
"INIT: Cannot open /etc/inittab errno: 2" And then the system boot into single user mode.
Please get detail information in boot.log attached.
I have such questions:
1. Since both the CDROM and the CD labeled "HP Tru64 UNIX Operating System V5.1B" are OK, but the CDROM can not recgonize the CD, does this mean that DS20E not support V5.1B?
2. If no for the first question, any suggestion to roll back (or continue) the upgrade? (We only have V5.1B CD, no V5.1 CD at the present.)

What, exactly, does this mean? Did the
machine say, "I can't recognize this CD"?
If not, what, exactly, did you do, and what,
exactly, happened when you did it?

Replace the CD-ROM drive? They do fail, you

No. First, while a DS20E is newer than _my_
Alpha hardware, it's not really all that new.
Is the CD-ROM older than the machine?
Second, if your Tru64 CD-ROM really were too
old for your DS20E, then you might not be
able to boot from that CD-ROM, but I can't
imagine anything (other than a bad CD-ROM
drive) which would keep you from being able
to mount the CD-ROM in Tru64.

If your unsupported upgrade scheme has done
some unknown damage to the OS, then I don't
know what would be the best way to proceed
(other than replacing the CD-ROM drive, and
trying a normal upgrade or fresh installation
from the CD-ROM).

We tried in two ways:
1. Mount in the OS:
# ls /dev/disk/cd*
/dev/disk/cdrom0a /dev/disk/cdrom0c
# mount /dev/disk/cdrom0a /mnt
Error: /dev/disk/cdrom0a is an invalid device or cannot be opened.
# mount /dev/disk/cdrom0c /mnt
Error: /dev/disk/cdrom0c is an invalid device or cannot be opened.
2. Try to boot from CDROM:

But it really looked like a solution and we had no other better options.

I just upgrade the firmware from the CD-ROM drive successfully. And also, I succeeded mounting the firmware driver CD.

As Steven suggests, your CD-ROM Drive is probably faulty. The DS20E shouldn't have any problems with 5.1B.

That's some evidence, but some CDs are more
reflective than others, and it's possible for
a CD-ROM drive to get either more dim or more
blind, and thus to have trouble with some
(less reflective) discs, while still working
well enough with other (more reflectve)
discs.
Also, home-made (CD-R, CD-RW) discs are
usually harder to read than factory-pressed
(aluminum-coated) discs. If your drive also
has trouble with these, that would be another
sign that it's going bad.
A replacement CD-ROM drive is so cheap (and
easy) to try, that I wouldn't hesitate.

Some old DEC/Compaq CD-drive cold not read CD-ROM written on speed more than 8-speed.
You cold try to copy 5.1B CD with slowest speed write.
The best way is prepare NHD CD too and install patch kit during installation run.
Some fixes conderning CD drive are there.

Thanks so much for your replies!
There should be something wrong with the CD Drive. We've tried to boot Tru64 5.1B on the other DS40 server and it boot up successfully.
So we decided to install the Tru64 5.1B on the DS40 server, instead of the DS20E desktop.
