SCSI CAM Error
We have 3 member MC cluster, suddently all 3 went down. I have tried to reconfigure the servers and setup looks like below.
We have 3 * DS20E having NCR53C895 UW SCSI controller which is connected to 70-33067-05 -Digital 2x68-pin Personality Controller Module on DS-BA356-KF Storage box connected thru BN21W-OB cable.
we used three Y cable to form three member cluster; looks like
Y |-------> DS-BAS356-KF storage box
server 1. ----|

Unfotunately we are getting following error whiel executing test bootstrap command or tried to install the OS on disk. But the servers are identified the DISKS [ RZ1DD-VW/RZ1CB-VW] which are all connected on BA356-KF box.

CAM LOGGER: SCSI evert pack
CAM LOGGER: SCSI bus 0

SCSI bus was reset
SCSI0: DIP intr, istat=0*9 dstat=0*81
CAM LOGGER: SCSI evert pack
CAM LOGGER: SCSI bus 0

SCSI: illegal instruction - DSP ******
SCSI0: can't find request for BA *****[proc SCSI Errors]

Hope I have provided sufficient informations and let me know the reason beyond this.

Check your logs and see what could be the source of the crash. To decode your crash information contact the HP Support Center.

not doesnot have sufficient informations since it was new installation. The console displayed the following message during fresh installation.
the error message was,

That is SCSI configuration problem (adresing, cables, settings ...)
I am not sure if I understood your hardware configuration well (you can attach a file with more informations).
What is the SCSI addreses of each HBA in every machine?
