E8212, slot B restarts regularly
This slot has a J9637A (Gig-T SFP module) which resets itself regularly; at least once a day. The switch log entry says that it loses the heartbeat and resets it.
The only recent operations were adding and removing a couple of SFP's, and I recently warm booted the switch to see if this problem will go away.
Has anyone else seen this behavior? If so, what is the best way to diagnose and/or fix it?

Put the suspect module on a known good slot, and a known good module to the suspect slot. Then check where the problem appears. Then save output of show tech all -command, and call your local support. They will take it up from there.
Just make sure that you don't power cycle the switch at any point before you've got the show tech all - this will clear the log.
