HP ProCurve Switch 5412zl module not coming up after power cut

Hello

We are having a problem with our Procurve  Switch 5412zl where a couple of the modules do not power up after a power failure.  I have to re-seat then and then all works fine. 

We have modules in A, B, C, D, E, F, G & J.  Not sure why H is not used, I think it was easier to run cables to J.

Has anbody else experienced this or can offer advise on how I can fix this.

Would be useful to know exactly:

• which Software Version (I suppose something like K.15.xx.yyyy) is your HP 5412 zl Switch currently running on.
• type and position of each Module it is equipped with.
• number and status of Power Supply (or Power Supplies) and status of Fans it has.
• features it provides (PoE, etc.).

Is the issue recurring each time you reboot the Switch or was it just a glitch?

Have you checked Switch's log during reboot (e.g. using Console or other methods)?
