You may need to contact MS for this and can follow the article on MS knowledge base.

One thing before you call MS is to update the system bios and the firmware of the Array controller if you have any. I have come across this issue several times and at times was able to fix it by updating the firmware of the Smart array controller. If you know which controller do you have then please let me know I shall post the link to update the firmware.

You need to make the first diskette only in both the cases and cold boot the system with it.
