BL25p shutdown

Hello,
We have a BL25p that randomly and suddenly shutdown. When this happens, we can't power it on using the ILO interface; in order to power it on, we have to take it out from the chasis and insert it again.

The last entry in the integrated managament log is "Server Blade Enclosure Rack Name Conflict".
Any idea?

any other server with same ip, or same serial number look in bios or same dns name

if in windows try from cmd try >>> wmic bios get / value sn number will be listed if windows about 10th from the bottom ..
